Working as a Zimbabwean Nurse or Healthcare Worker in the UK: NMC Registration, OET, and NHS Routes

Zimbabwe has produced generations of highly trained healthcare professionals, many of whom have built successful careers in the UK's National Health Service. The pathway from Zimbabwean nursing qualification to UK registration involves several structured steps, and understanding the full process — including recent policy changes that have significantly altered the landscape — is essential before investing time and money.
## The Scale of Zimbabwean Healthcare Migration
Nearly 20,000 Zimbabweans were granted UK Health and Care Worker visas between 2021 and 2024, making healthcare one of the dominant routes for Zimbabwean migration to Britain. In 2025, around 75% of the 841 UK work visas granted to Zimbabweans came through the Health and Care Worker route. However, the total figure represents a 68% decline from 2024, reflecting significant tightening of UK immigration policy for this sector.
## Critical Policy Changes You Must Know
The UK government has fundamentally restructured its approach to overseas care worker recruitment. As of 22 July 2025, care workers and senior care workers were removed entirely from the Skilled Worker route. This affects those aiming for roles in the social care sector specifically. Additionally, since March 2024, care workers and senior care workers are no longer permitted to bring dependants to the UK. The government has signalled plans to phase out overseas care worker recruitment altogether as part of broader immigration reform.
These changes do not affect registered nurses and qualified clinical healthcare professionals in the same way. The Health and Care Worker Visa remains available for registered nurses, midwives, doctors, and allied health professionals — but the route is considerably more restricted than it was in 2022 and 2023.
## NMC Registration: The Essential Gateway for Nurses
To work as a registered nurse in the UK, you must join the Nursing and Midwifery Council (NMC) register and obtain an NMC PIN. No employer can legally engage you as a nurse without this. The process has several distinct stages.
**Stage 1: Initial Application and Document Submission**
Create an account on the NMC online portal and submit your application with the required fee. You will need to upload your nursing qualification certificates, a valid Zimbabwean passport, your home country nursing licence or registration, a Good Standing Certificate from the Nurses Council of Zimbabwe, a police clearance certificate, and health and character declarations.
**Stage 2: English Language Proficiency**
You must demonstrate English language competency through one of the accepted routes. The two primary test options are:
- **IELTS Academic**: A minimum overall score of 7.0, with no individual component below 6.5, except writing which must be at least 6.5.
- **OET (Occupational English Test)**: A minimum of grade B in three components and at least C+ in writing. The OET is specifically designed for healthcare professionals and tests medical and clinical communication, which many nurses find more relevant to their work.
If you completed your nursing training entirely in English, or have worked for at least a year in a predominantly English-speaking country, you may be able to provide alternative evidence — but this is assessed case by case.
**Stage 3: Computer-Based Test (CBT)**
Once the NMC verifies your documents and issues a Decision Letter, you are authorised to sit the CBT. This is a theory-based multiple choice examination that tests nursing knowledge and can be taken online in Zimbabwe before you travel to the UK. Passing the CBT allows you to proceed to the OSCE.
**Stage 4: Objective Structured Clinical Examination (OSCE)**
The OSCE is the practical stage and must be completed in person at one of five approved test centres in the UK. It comprises ten clinical stations covering patient assessment, medicines administration, communication, infection control, and other core nursing competencies. It follows the APIE framework (Assessment, Planning, Implementation, Evaluation). The OSCE tests safe, structured patient care in a simulated UK clinical environment — it rewards clinical confidence and communication skills rather than rote memorisation. Many candidates who pass the CBT comfortably underestimate the OSCE. Preparation with other nurses who have recently passed is widely considered the most effective approach.
**Stage 5: Final Registration and PIN**
After passing both exams, you submit final paperwork, pay the registration fee, and receive your NMC PIN. This is the point at which you are officially a registered nurse eligible to practise in the UK.
Most Zimbabwean nurses arrive in the UK on conditional NMC registration, complete the OSCE within their first weeks, and their employer typically arranges the booking.
## The Health and Care Worker Visa
Once you have a job offer from a licensed sponsor, your employer issues a Certificate of Sponsorship (CoS). You then apply for the Health and Care Worker Visa online. The visa fee is currently £247 for placements under three years and £479 for over three years — these are reduced rates compared to the standard Skilled Worker visa. You must also demonstrate maintenance funds of at least £1,270 held in a bank account for 28 of the 30 days before applying, unless your employer agrees to cover your first month's costs, which they can note on the CoS.
Zimbabwean applicants must also complete a tuberculosis test at a UK Visas and Immigration approved clinic in Zimbabwe. Results are typically available within a few days and are valid for six months.
Visa processing for Health and Care Worker applications is typically decided within three weeks. Priority processing is available at additional cost.
Your English language test certificate from OET or IELTS satisfies the visa's English language requirement — you do not need a separate English test for the visa itself if you already have one for NMC purposes.
If travelling with family, additional maintenance funds are required: £285 for a partner, £315 for your first child, and £200 for each additional child. Note that as of March 2024, care workers (SOC codes 6145 and 6146) can no longer bring dependants — this restriction does not apply to registered nurses.
## Settlement and Long-Term Prospects
After five continuous years in the UK on a Health and Care Worker Visa, you can apply for Indefinite Leave to Remain (ILR). The salary threshold for settlement is £31,300 per year or the going rate for your occupation code, whichever is higher.
## Finding Sponsored NHS Jobs
The NHS Jobs portal and NHS Trust websites advertise internationally sponsored positions. Many NHS Trusts actively recruit from Zimbabwe through formal international recruitment programmes. Agencies with NHS contracts also facilitate sponsored placements. When evaluating any offer, confirm the employer holds a valid sponsor licence and is registered with the Care Quality Commission where relevant.
## Protecting Yourself
Costs mount quickly: English language tests, the NMC application fee, CBT and OSCE preparation, a TB test, and police clearance collectively represent a significant financial commitment before a single pound of UK salary is earned. Be cautious of agents charging large upfront fees with vague promises. Legitimate NHS employers and reputable recruitment agencies do not typically require large payments from candidates. The exploitation of overseas healthcare workers under sponsored arrangements has been widely documented and prompted the government policy changes currently in force.
